Good Faith Exam
As of 2026, Arizona law requires patients to complete a Good Faith Exam (GFE) before receiving certain medical aesthetic treatments. This medical evaluation must be completed by a licensed healthcare provider to establish a provider-patient relationship and determine that your treatment is safe and appropriate for your individual health needs. If your Good Faith Exam has expired (more than 12 months old), you must complete a new exam before your treatment appointment. Failure to do so may result in your appointment being rescheduled until this requirement has been met.

The evaluation establishes a provider-patient relationship before prescription medications or medical devices are used and helps ensure your treatment is both appropriate and medically safe.
Treatments That Typically Require a Good Faith Exam
A Good Faith Exam is generally required before treatments such as:
Botox® and other neurotoxin injections
Dermal fillers
Laser skin rejuvenation
Laser hair removal
IPL (Intense Pulsed Light) treatments
RF Microneedling

Other physician-directed aesthetic treatments involving prescription medications or medical devices
